Showers, Blossoms, Babies, and Bleach
Greetings from the dental office! Spring has made its presence quite obvious outside lately. First crocus and now daffodils, forsythia, and magnolias are blooming here in the Spokane Valley. The rhubarb is up and trees all around us are budding out and ripe for a foliage explosion.
At our house, we have had 3 baby lambs and 4 baby goats born in the last 3 weeks, one new doeling last night. In the next two weeks, what will spring bring? I expect to see hummingbirds and serviceberry blossoms. Who will be the first to spot these landmarks of spring.
And, what are dental patients looking for? Well, as people do their spring cleaning inside the house and outside in the yard, to make the house and property look great, likewise, people want to have their smile look great also. Whiter teeth is a frequent request all year, but more so in the spring. Many people may be looking forward to graduation, reunions, weddings, and whiter teeth may be on their mind. But, whatever drives the desire, we can whiten teeth.
Just a couple caveats to consider before bleaching teeth:
- Whitening toothpastes are really a con – don’t buy into it. What is done during and after bleaching is a big determiner of success vs. failure.
- Teeth are covered with enamel rods. Despite the fact that enamel is the hardest substance in the human body, under an electron microscope enamel is quite porous. It looks like PVC pipe stacked in a pile, from the end you see all the tubes. When teeth are bleached, all the coffee, tobacco, tea, blueberry, etc. stains are cleaned out of those tubes. Following bleaching, the teeth reflect light better and look both whiter and brighter.
- Once teeth are bleached, those now clean tubules need to avoid all staining colored liquids, not forever, but for at least 2 weeks. This gives these newly bleached enamel rods a chance to fill in with clear fluid. This interim period must be gone through without cola, tea, coffee, huckleberry sauce, apple juice, etc. If you can’t give up coffee, tobacco, etc. for two weeks, don’t consider bleaching teeth. Anything with color can and will re-stain those freshly bleached tubules.

Dentistry in a Struggling Economy

Dentistry in the United States is second to nowhere in the world in terms of quality. We are fortunate in this great nation to have not only highly trained dentists and oral health specialists, but to have their services readily available as well. However, in these difficult economic times, many people are not taking advantage of the excellent dental care available.
On a regular basis people tell me, “I don’t have dental insurance, so I can’t have dental work done.” However, oral health is crucial to overall health and quality of life and should be considered a necessity, not a luxury. Many of my patients would prefer to complete diagnosed dentistry no matter the cost, no matter whether or not they have insurance coverage. Mind you, I’m not saying that insurance isn’t often a significant help—it is. However, your dental work should not be governed by an insurance company. Each person should spend time with his or her dentist to discuss how to make their dentistry affordable for their situation. Everyone should know what good oral hygiene is and receive instruction in how to practice preventive dentistry. Your dentist and dental hygienist are a wealth of knowledge on how you, as an individual, can best keep, maintain, and restore your oral health.
Oral hygiene is best when practiced regularly and frequently. It should be easy to understand that a person who brushes their teeth twice a day and flosses daily will enjoy better, more stable oral health than a person who brushes infrequently or not at all. Machines run better and last longer when well-maintained; likewise, patients who come in for a twice-yearly professional teeth-cleaning and check up complete with oral cancer screening rarely need restorative dentistry. However, those who do not practice any preventive dentistry are most often seen on an emergency basis only.
I see patients from all walks of life, and I can tell you from experience that the healthiest mouths are not found only in those who can easily afford quality dentistry. The healthiest mouths are found in people who understand and practice preventive dentistry, no matter their income. Frequently, patients cannot afford to have all the necessary, diagnosed dental work to be performed at once. However, we can almost always complete a treatment plan in which the dental work is done in phases for those who truly want to have quality dentistry.
The point is, don’t let financial difficulties or a lack of dental insurance prevent you from taking proper care of your oral health. Preventive dentistry in the form of regularly brushing and flossing will go a long way toward ensuring that dentist appointments are simple, inexpensive affairs, and when you do need more extensive dental work, don’t put it off until it becomes an emergency. By discussing it with your dentist, you can work out a treatment plan that will get you back on the road to full health in a way that won’t burden you financially. What Do We Deserve?
Summer is at its peak. Garden vegetables and fruits are beginning to generously grace our tables. After a long, cold spring, it’s nice to have warm days of summer in Spokane. Sun bathing and bathing in pools and water parks are suddenly popular. The price of gasoline may have shortened vacations, but everyone deserves to enjoy the long-awaited temperature increases.
Likewise, everyone deserves to have a smile they enjoy showing off.
Within the last week, I’ve had a gentleman in his mid-30s come to me for dental care. He has three upper front teeth missing and multiple teeth with decay in them. Needless to say, this very nice looking gentleman avoided smiling and was ashamed of his mouth. Appearance can raise or lower one’s self-esteem. In this case, our “John Doe” was daily humiliated (or so he felt) by his toothless smile. Further complicating his situation was the fact that he had an exaggerated gag reflex and an extreme fear of dentists. In his favor, he had most of his original set of teeth and, despite the rampant decay, his bone support was very good so his mouth was very restorable.
John Doe was interested, most of all, to get his smile back.
John Doe was so distraught he actually began to cry in the dental chair. Because of the amount of dentistry needed in this case, we recommended oral sedation.
The advantages to this patient in using oral sedation are multiple. We can do much more work in a single sitting. We can make him more comfortable by stopping the gag reflex. He will not shake in fear as he did on the initial exam. Because he’ll be much more relaxed during each dental visit, his post-operative discomfort will be drastically reduced. And most importantly, he’ll have a new smile in just weeks, having gone years with a smile he was ashamed of. Soon he’ll enjoy the warm weather with a new smile.

Fear of the Dentist and Sedation Dentistry

Yesterday I had a lady in my dental chair who said she gets heart palpitations just thinking about coming to see me. In a different context, that might make me feel “macho,” but in reality, she simply has a fear of dentists. Most of us have a fear of something.
